Our Legal Duties
We are required by federal law (HIPAA) to maintain the privacy of your protected health information (PHI), provide you with this Notice describing our legal duties and privacy practices, notify you promptly if a breach occurs that may have compromised the privacy or security of your PHI (no later than 60 days after discovery), and follow the terms of this Notice until it is replaced or revised.

 

How We May Use and Disclose Your PHI
We may use or disclose your PHI without your written authorization for the following purposes: – Treatment ‒ To help you obtain health coverage and coordinate benefits. – Payment ‒ To obtain payment or reimbursement for services. – Health Care Operations ‒ For quality assessment, training, compliance reviews, and audits. – Required by Law ‒ When required by federal, state, or local law. – Business Associates ‒ With vendors or contractors who perform services on our behalf under HIPAA-compliant agreements.

 

Other Uses and Disclosures Requiring Authorization
Any other use or disclosure of your PHI not described in this Notice will be made only with your written
authorization. This includes marketing communications not related to your insurance plan and the sale of PHI. You may revoke an authorization at any time by providing written notice to us, except to the extent we have already acted in reliance on it.

 

Your Privacy Rights
You have the right to request access to and a copy of your PHI, request corrections, receive an accounting of certain disclosures, request restrictions on use or disclosure, request confidential communications, and obtain a paper copy of this Notice at any time.

 

Our Safeguards
We protect your PHI using administrative, physical, and technical safeguards and will use or disclose only the minimum amount of PHI necessary to accomplish the intended purpose. If you choose to communicate with us by email or text, please be aware that these methods may not be fully secure. By using these methods, you accept that risk.

 

Changes to This Notice
We reserve the right to change our privacy practices and this Notice at any time. Any revised Notice will apply to all PHI we maintain and will be posted in our office and on our website.
